Procedures that need to be completed and submitted to the coach.
1. Participation Fee paid in full and receipt for payment turned into the coach. Please DO NOT pay the fee until your student athlete has officially made the team. See above for link to pay.
2. Participation Agreement must be read and signed by student-athlete and their parent/guardian and turned into the coach on the first day of practice. See the Participation Agreement tab on the left side of this page to access the packet.
3. Annual Physical and Permission Form must be completed (signed by both the parent/guardian and Physician) and turned into the coach. This must be completed after June 1 for the current school year. This also needs to be done before the first day of practice. See the Athletic Physicals tab on the left side of this page to access the paperwork. If you had a physical and participated in a Fall sport and plan on participating in a Winter sport, all you need to complete is the re-certification form (parent completes, not physician). If their was an injury or concussion that occured after the initial Physical, the re-cert form needs to be filled out by the parent and physician to be cleared for participation. You can access the re-cert form in the physical packet.

A parent must be present in order to get the physical.
If your student has not had a physical after June 1, 2019 he/she may get one at East HS. All Spring athletes must present their physical in order to try out. If this is the second sport for the 19-20 school year, please complete the re-certification form under the "physicals" tab.
Dr. Frank Brennan from Brandywine Village Family Medicine will be conducting the physicals. The cost of the full physical in $25. Cash is preferred. Checks are to made payable to "Dr. Francis Brennan".
If a re-certification physical is needed (due to injury during first sport) the charge is $20.
