    DRIVE THRU FLU SHOTS

     

    Flu Shots will be available for all staff at the Administration Building located at 540 Trestle Place on

     

    Tuesday, September 29 Between 2:30pm and 4:30pm

    Thursday, October 1, 2020 Between 2:30pm and 4:30pm

     

    The Flu Shots will be administered on-site by Rite Aid Pharmacy. 

    You will just drive up, turn in your completed Consent Form to the pharmacist, and get your flu shot.

     This year will be different. You will not leave your car.

     

    What do I need to do?

     

    1.) In order to receive a flu shot on September 29, please register by clicking here by September 25.

    (Please do not sign up again if you are already registered for October 1)

     

    2.) Complete Consent Form and bring it with you.

     

    3.) VERY IMPORTANT – Write your complete Member ID on back of the Consent Form. If you have DASD insurance, the number starts with DWU…

     

    If you have DASD medical insurance, there will be no need to bring money with you and no need to submit for reimbursement.  

    The claim will be processed by the pharmacy with the insurance, but you must write the Member ID Number on the form. 

     

    If you do not have DASD insurance, you can still participate by paying the fee for a flu shot or by submitting your own insurance. 

    However, you should check with your provider on flu shot payment information beforehand.
